Before submitting: Applications without an organization budget, proof of federal tax exemption, or project budget are incomplete.

Required materials

  • Contact name, phone, and email
  • Proof of federal tax exemption
  • Current organization budget
  • Balance sheets and income statements for the prior two years
  • Detailed project budget

Submit your proposal

  1. Submit by 3 p.m. on the deadline date.
  2. Complete every section and attachment.
  3. Be concise; addenda are accepted.
  4. Email to office@theoconnorfoundation.org. Mailing is accepted but not preferred.
Mail preparation: Use 8½ × 11-inch paper, one side only, and do not staple or bind.
